- If you are a healthcare provider applying to join our Lupus Therapeutics Healthcare Provider Directory™, we also collect:
- Your professional information, including your credentials
- Your employer or practice name
- Information on the health plans you or your employer/practice accepts
- Any other information you choose to provide us
It is always your choice to provide us with this information. Some of this information, however, is necessary for you to participate in certain activities or events (such as to receive information you have requested).
